By agreeing to come along to our Summer Camps you agree to adhere to the following guidelines:
Netball NI takes our responsibility seriously to do all we can to minimise the risk of spreading coronavirus whilst providing a safe and fun environment for our participants and coaching team.
Coronavirus Related Health Check
I understand, to play our part in minimising the risk of spreading coronavirus, that my child will be unable to participate in a Netball NI Summer camp if she or anyone she has been in contact in the last 14 days has tested positive for coronavirus, or displayed symptoms of coronavirus such as
- flu like illness,
- a temperature of 37.6 or higher or
- a loss of taste/smell
I take full responsibility to inform Netball NI of any changes relating to the health of my child in the lead up to Summer Camp.
I agree to complete a health questionnaire on each day of camp, and understand that my child will be asked to take a temperature check (using a non contact thermometer) on arriving at the camp venue. A temperature of 37.5 or below is required to be able to participate in a Netball NI Summer Camp.
I recognise these procedures are only in place to ensure the safety of my child, the other participants and Netball NI coaches, and to assist with Netball NI’s Return to Play protocols with regards to coronavirus.
Statement of General Health
My child is in good health, and I consider her capable of taking part in the Netball NI Summer Camp.
I have shared any medical details and consent that, in the event of illness, having parental responsibility for my child, I give permission for medical treatment to be administered where considered necessary by a nominated First Aider, or by suitably qualified Medical Practitioners. If I cannot be contacted and my child should require emergency hospital treatment, I authorise a qualified Medical Practitioner to provide emergency treatment or medication that may include the use of anaesthetics can be administered to my child/young person. I also understand that while Coaches and Netball NI personnel will take every precaution to ensure that accidents do not happen, they cannot necessarily be held responsible for any loss, damage or injury suffered by my child.
I am responsible for making arrangements to get my child to and from the camp venue, and I understand that Coaches are not responsible for my child once they leave the venue.
I have been made aware that Netball NI has a Safeguarding Policy, available on the Netball NI website http://www.netballni.org/safeguarding/
Use of Photography
I confirm my child has permission to participate in any official media including television, photography, print and social media.
GDPR and Data Protection
I agree for Netball NI to
- hold my data on a central database in line with their retention and disposal policy and schedule
- to contact me with regards to netball camps and other netball opportunities
- for my details to be shared with programme funder body Sport NI adhering to Data Protection law.
I understand I can request the removal of my details at any time by contacting firstname.lastname@example.org
